并州城牢狱坍塌、突发鼠患，狱卒被鼠群围攻离奇死亡，死刑犯被关押做神秘实验，城中名医苦求父母官治理鼠疫，当地太守却莫名不作为。大量村民因为鼠患而染病，被困城中，为了挽救全城百姓的性命，狄仁杰（陈浩民饰）临危受命、追查病源，诸多线索千头万绪，鼠患灾难迫在眉睫，而在层层迷雾之中，狄仁杰抽丝剥茧、激浊扬清，不断盘查病源，寻找真相，然而随着调查的深入，原来鼠灾背后竟然还隐藏着一个巨大的阴谋。
